The creation of a custom skybox is only limited to your knowledge of photoshop. You'll need the vtfplugin (http://nemesis.thewavelength.net/index.php?c=155), and if you are wanting an hdr skybox you'll need to do alot more in the way of layers and how they are saved. I'll try and get a tutorial up soon, as i've been asked several times on the subject.

Only the HDR bit is hard. I can't find a decent tutorial that actually explains whats going on, just how to do it...

nope not at all.
They are limited to powers of 2, that means 2^2 2^3 2^4 .... that works out as 2,4,8,16,32,64,128,256,512,1024,2048. The sides of your texture can use any of those, so you could have 128x256 or 512x512 (which the sky texture are) or 1024x64....
